Hyperparathyroidism is a rare disease that affects the whole body and can cause potentially debilitating symptoms. It is often poorly understood, even by parathyroid specialists. The only treatment is surgery, which has a very high success rate when performed with precision and planning by a surgeon with extensive experience not only in parathyroid surgery, but also in surgery of the entire neck. Parathyroid problems can be very complex, as the signs and symptoms are similar to other diseases, so patients are often misdiagnosed. Every patient needs to be individually evaluated to make sure the diagnosis is correct, as laboratory tests can be confusing and ultimately mislead both the patient and their physicians. A great number of our patients are told that they don’t have a medical condition, and their symptoms are due to aging, stress, depression, menopause, fibromyalgia, etc. We give our patients the time and attention they deserve to make sure they are properly diagnosed, so we can begin to help them return to optimal health.
Once diagnosed, we have to identify which one of the four parathyroid glands is abnormal so that we can perform a focused minimally invasive surgery. Our philosophy is to start with the most minimally invasive and least potentially harmful scan, which is the neck/thyroid ultrasound. A great majority of abnormal parathyroids are identified through the use of in-office ultrasound performed by the surgeon, Dr. Larian. If the ultrasound does not locate the abnormal gland then we proceed to use either the sestamibi/SPECT Scan or 4D high resolution CT scan. Both these scans are complex, require experience, and have radiation exposure, which is why they are only used when the ultrasound is negative. Our primary goal is to give each patient the personalized care and attention needed to cure hyperparathyroidism.

Dr. Larian completed a six-year residency program in Head and Neck Surgery at UCLA, where he had intense training in treatment of head & neck tumors. This training focused on diagnosing and managing complex tumors and cancers in the neck, the complications caused by these tumors, and repairing these complications. This experience allowed him to have a deep and extensive understanding of not only the anatomy of the neck, but also consequences of disease in this very delicate area of the body.
A passion for treating parathyroid disorders naturally developed from this experience, as well as a result of seeing what lengths parathyroid patients often times have to go through to ultimately be diagnosed properly and then receive appropriate and effective treatment. Whereas a great majority of parathyroid tumors are easily identified and removed, some tumors can be located anywhere in the neck, and a certain percentage of patients have disease in multiple glands. As a result of this potential complexity, parathyroid surgery requires immense experience, patience, and thoroughness. THERE IS NO ONE TECHNIQUE THAT WORKS FOR EVERY PATIENT. As such, we specialize in Minimally Invasive Parathyroidectomy (MIP). With its infinite advantages, it has become the preferred method of operation for parathyroid glands. Although it is often thought that minimally invasive surgery simply means a smaller incision, in terms of MIP this also means less disturbance of the tissue under the skin. Scar tissue will form in any area of the body that is operated on, and scarring can have long term consequences in the neck; as we speak and swallow the voice box, trachea (breathing tube), and the area around the thyroid move up and down, and scarring can interfere with this important mechanism. Therefore, it is vital the surgeon takes great care when operating on the parathyroid glands and does not interrupt or damage this delicate mechanism. Additionally, the more extensive the surgery is, the greater the risk to the recurrent laryngeal nerves and dormant shrunken parathyroid glands. A truly minimally invasive parathyroidectomy with Dr. Larian aims to minimize all of these risks, while considering all the possibilities of parathyroid disease (double adenomas or hyperplasia), and utilizes all the techniques available to completely cure the patient (including intra-operative PTH testing, radio guidance, and potentially comprehensive neck exploration or 4 gland exploration, when necessary).
